Overview
Administered by COAAA’s Clinical Innovations team, Veteran-Directed Care (VDC) ensures that veterans receive needed in-home services and empowers the veteran to determine who provides that care.
Referrals for VDC come to COAAA through the local Veterans Affairs Medical Center (VAMC). The VAMC determines eligibility. Veterans who may be eligible include a veteran of any age, one who is enrolled in VA healthcare and wants to live in their own home, meets a clinical need for VDC, and is interested in directing their own care. Often times, the paid care provider is a family member.
The VAMC approves the monthly spending budget and COAAA provides the ongoing case management and coordination of VDC in-home services. The care provider directly administers the in-home services, which can include meal preparation, personal care, homemaking, medication administration, and more.
