PASSPORT provides help at home for seniors as an alternative to nursing home care.

Overview

Referrals to PASSPORT home-care program may be made by calling PASSPORT Intake at 614-645-7250 and selecting the Screening option.

Examples of services available include personal care services, home-delivered meals, emergency response systems, adult day services, homemaker, transportation, and case management.

PASSPORT, providing help at home for Ohio seniors, is also referred to as Ohio's home and community-based Medicaid waiver program for older adults.

Eligibility

To be eligible, a person must be 60 or older, be financially eligible for long term care Medicaid, and have qualifying functional needs which require a nursing home level of care. This would be determined through a formal medical/functional evaluation. Applicants must be able to remain safely at home with the approval of their primary care provider.

In addition, those who are eligible for PASSPORT in Ohio receive a Medicaid card to access benefits, including the cost of medications and health care.
