Choices Program Brochure

Choices is a consumer-directed homecare option available only in Central Ohio, for PASSPORT consumers served by COAAA. Choices is a model waiver program, with the same eligibility criteria as PASSPORT. Choices provides the consumer the opportunity to become the employer and hire individual providers, such as friends, neighbors, or relatives (with some exceptions) to provide their home care. In this role the consumer assumes greater control over delivery of services, as well as the service providers. Enrollment in the program is limited to 200 consumers. The first Choices consumer was enrolled on February 1, 2002.

The Choices Program is part of The Robert Wood Johnson Foundation’s Independent Choices Grants (1997). Scripps Gerontology Center in conjunction with the Central Ohio Area Agency on Aging received funding to evaluate the Choices Program.

PASSPORT receives 99% approval rating

Consumers gave Ohio’s 12 Area Agencies on Aging (AAAs) a better than 99% approval rating on their management of the PASSPORT program throughout 2011.  On January 30, the Ohio Department of Aging released data reporting results of its consumer satisfaction survey for the entire state.

Consumers rated their satisfaction through participation in programs offered by AAAs at over 99% statewide.

“We are passionate about providing information to individuals and family members and helping them navigate through a complex system of care when they need it most," said Central Ohio Area Agency on Aging Director Cindy Farson.   

"Our employees work very hard to respond to the needs of frail people who rely on in-home services, like PASSPORT, to remain in their communities.”
