Choices Program Brochure

Choices is a consumer-directed
homecare option available only in
Central Ohio, for PASSPORT consumers
served by COAAA. Choices is a model
waiver program, with the same
eligibility criteria as PASSPORT.
Choices provides the consumer the
opportunity to become the employer and
hire individual providers, such as
friends, neighbors, or relatives (with
some exceptions) to provide their home
care. In this role the consumer assumes
greater control over delivery of
services, as well as the service
providers. Enrollment in the program is
limited to 200 consumers. The first
Choices consumer was enrolled on
February 1, 2002.
The Choices Program is part of The
Robert Wood Johnson Foundation’s
Independent Choices Grants (1997).
Scripps Gerontology Center in
conjunction with the Central Ohio Area
Agency on Aging received funding to
evaluate the Choices Program.
