  • Medicaid-waiver eligible
Home First Nursing home residents bypass any potential PASSPORT managed enrollment if they wish to live in the community.
  • Transition from a long-term care facility to community-based care such as PASSPORT, Assisted Living and Residential State Supplement
  • Age varies **
  • Currently in a long-term care facility

  • Same level of assistance as PASSPORT

** Residential State Supplement: 18+; Assisted Living: 21+; PASSPORT: 60 or older.

Choices
The consumer may become the employer and hire individual providers, such as friends, neighbors, or relatives (some rules apply) to provide care.

Assisted Living
Waiver
Provides care for Medicaid-eligible adults in approved assisted living facilities.  An alternative for people needing more care, but not a nursing home.
